In consideration for being permitted to participate in voluntary prayer ministry, herein referred to as “Desert Deliverance,” I the undersigned, do hereby release and discharge Desert Deliverance, any minister(s) duly appointed by Desert Deliverance, their heirs, executors, administrators, successors and assigns from all action, causes of actions, suits, claims and demands whatsoever in law or equity which I or my personal representatives, insurer, spouse and next of kin, heirs, executors, administrators, successors, and assigns now have, or may have in the future by reason of any matter or cause whatsoever arising from, related to, or in any way connected with the assessment for demonic oppression or deliverance prayer, and performance of the said deliverance prayers upon me.
